SSS Maternity Benefit Reimbursement Requirements for Voluntary, Self-Employed, and Separated from Employer Members
Below is the complete list of requirements when filing your maternity benefit reimbursement with SSS:
**Please note that this is for members who are unemployed (voluntary), self-employed, and separated from employers
1. SSS Maternity Benefit Reimbursement Application (MBRA) Form for members who are Self-Employed, Voluntary, or Separated from Employer. Download the form here: https://bit.ly/SSSMat2SE
2. Maternity notification.
- Mat 1 form duly received by SSS before delivery (for those who filed via SSS offices). Download it here: https://bit.ly/SSSMat1
- Maternity Notification Submission Confirmation (for those who filed online)
3. Photocopy from the Original or Certified True Copy of the following documents (whichever applies to you):
- Normal Delivery:
- Child's birth or fetal death certificate duly registered with the LCR
- Caesarian Delivery:
- Child's birth or fetal death certificate duly registered with the LCR
- Any of the following documents issued by the hospital indicating the type of delivery:
- ORR (Operating Room Record)
- Surgical Memorandum
- Discharge Summary Report
- Medical/Clinical Abstract
- Delivery Report
- A detailed invoice showing CS delivery charges (For deliveries abroad only)
- For Complete Miscarriage:
- OB History indicating the number of pregnancy/pregnancies duly certified by attending physician with Professional Medical License number, printed name, and signature.
- Any of the following:
- Pregnancy test before and after miscarriage
- Ultrasound report indicating proof of pregnancy
- Medical Certificate issued by attending physician in the circumstance of pregnancy
- For Incomplete Miscarriage:
- OB History indicating the number of pregnancy/pregnancies duly certified by attending physician with Professional Medical License number, printed name, and signature.
- Any of the following:
- Certified True Copy of hospital/medical record/s
- D & C (Dilation & Curettage) report
- Histopathological report
- Pregnancy test before and after miscarriage
- Ultrasound report indicating proof of pregnancy
- For Ectopic Pregnancy:
- OB History indicating the number of pregnancy/pregnancies duly certified by attending physician with Professional Medical License number, printed name, and signature.
- Any of the following:
- Certified True Copy of hospital/medical record/s
- Certified True Copy of ORR (Operating Room Record)
- Histopathological report
- Pregnancy test before and after miscarriage
- For Hydatidiform Mole (all of the following):
- OB History indicating the number of pregnancy/pregnancies duly certified by attending physician with Professional Medical License number, printed name, and signature.
- D & C report
- Histopathological report
4. Photocopy from the Original or Certified True Copy of the following documents (whichever applies to you):
- Previously employed:
- If delivery/miscarriage/procedure done is within employment or within six months from date of separation:
- Certificate of separation from employment indicating the effective date of separation and that no advance payment was granted (signed by employer's authorized signatory as reflected in SS Form L-501).
- Separated from employment:
- If delivery/miscarriage/procedure done is within employment from the date of separation:
- Certificate of separation from employment indicating the effective date of separation and that no advance payment was granted (signed by employer's authorized signatory as reflected in SS Form L-501).
- If delivery/miscarriage/procedure is after the date of separation:
- Certificate of separation from employment indicating the effective date of separation (signed by employer's authorized signatory as reflected in SS Form L-501).
- Certificate of separation is not required for below circumstances but you need to submit supporting documents as required:
- Company is on strike:
- Notice of strike duly acknowledged by DOLE
- A duly notarized Affidavit of Undertaking issued by the member that no advance payment was granted and with the indicated date of separation.
- Company has been dissolved or ceased to operate:
- A duly notarized Affidavit of Undertaking issued by the member that no advance payment was granted and with the indicated date of separation.
- There is a pending case before a court regarding the separation of member:
- Certification from DOLE
- A duly notarized Affidavit of Undertaking issued by the member that no advance payment was granted and with the indicated date of separation.
- If AWOL or with strained relations with the employer:
- A duly notarized Affidavit of Undertaking issued by the member that no advance payment was granted and with the indicated date of separation.
5. Filer's SSS card or 1 Primary ID/Government ID or 2 Supplementary IDs.
